A partnership program with health-care professionals and the Alzheimer Society of Toronto to ensure that individuals and families dealing with dementia receive the support and information they need throughout the course of the disease * seminars, workshops and training sessions * advanced care planning for future health care needs

About Us

First Link is the resource for healthcare professionals to connect individuals and families with new dementia diagnoses with the Alzheimer Society of Toronto. First Link social workers connect with families earlier through this program. Staff connect clients to resources and supports provided by the Alzheimer Society of Toronto and other community services.
