Description of Services:

Education, Self-Management and Exercise Program for adults with history of cardiac disease

Offers a 3-month program
Healthy Lifestyles in Thunder Bay provides assessment, stress test, and prescription prior to local participation
Program offered through Telemedicine connection to Thunder Bay for education sessions, exercises locally
Allows 4 people per class (limited to exercise equipment in physio department)
