Services that guide those with physical disabilities to independency * self-directed services * personal support and homemaking services are available * outreach services can be customized to fit the environment that is needed: home, place of employment or educational institution

meal preparation
housekeeping
assistance with shopping, banking and appointments
involvement in scheduling outreach services
nurturing services for parenting assistance
assistance with mobility

If an individual requires the assistance of a Power of Attorney (POA) or Substitute Decision Maker (SDM) that individual will not qualify for this self-directed program
