Description of Services:
The role of the hospital care coordinator is to assess and arrange for required Ontario Health atHome in-home services, and/or placement services, upon discharge from the hospital and to link patients to other useful community resources. In-home services may include personal care, nursing, physiotherapy, occupational therapy, speech language pathology, nutrition and social work.
